POSITION SUMMARY: Oversee Day Habilitation programs to ensure compliance with accreditation, auditing, and organizational standards. Provide active supervision and instruction of staff in the development and implementation of the CCSP & MCO Plan of persons served.
ESSENTIAL JOB FUNCTIONS:
· Ensure quality and timeliness of Day Habilitation services. Ensure necessary supplies are maintained. Assist with on-site training and orientation of persons served.
· Analyze area monthly financials and proposes modifications as assigned by Day Program Administrator.
· Lead area staff to manage and administer programs and services consistent with licensure and accreditation bodies and evaluates the results of overall operations regularly and systematically and reports these results to the Day Program Administrator.
· Interpret policies and procedures to area personnel.
· Provide supervision to all department personnel including coaching, the development and review of performance evaluation, completing the interview process and proposed salary adjustments. Compile information and consult with Department Director for preparation of disciplinary actions as needed and participate in the disciplinary actions as coordinated with the Corporate Operations Director.
· Complete assigned responsibilities for the formal program evaluation and preparation of surveys.
· Represent Day Habilitation programs and Link Associates to internal and external stakeholders, persons served and the community in a professional manner.
· Conduct routine staff meetings, team building sessions, and individual meetings with staff to review goals and Life Plans of persons served.
· Confer with persons served and stakeholders to evaluate, market, and promote services.
· Assist in the development of the CCSP for persons served as assigned, including pre and post staffing paperwork and report on-going progress on goals. Punctually and professionally attend meetings for persons served as assigned.
· Assist with the development, implementation, and oversight of curriculums.
· Instruct and counsel persons served as needed.
· Supervise and provide coaching to assigned personnel to ensure the CCSP & MCO Plan for persons served are implemented and progress is being made and recorded.
· Ensure documentation for persons served is complete, accurate, and represents individual goal obtainment. Perform internal reviews of documentation as directed.
· Assist in ensuring that the files of persons served are kept according to standards.
· Complete and submit as instructed all required documentation in a manner that is legible, accurate and inclusive of all required information.
· Assist Day Program Administrator and other staff with projects and other duties as assigned and coordinate area activities with those of other areas and departments.
QUALIFICATIONS:
· Bachelor’s degree in Education, Social or Human Services with two (2) years related successful work experience and one (1) year of supervisory experience. Exceptions must receive prior approval from the Executive Director.
· Must meet agency policy on transportation, background checks and provide on-going proof of personal vehicle insurance.
· Medication Manager Certification or Certified Nurses Aide Certification preferred.
· Possess ability to work on a daily basis utilizing standard shop, home and office equipment.
